Option One: An original of any one of the following documents may be provided to prove identity and employment eligibility: 

• Untied States Passport
• Certificate of U.S. Citizenship  (INS Form N-560)
• Certificate of Naturalization  (INS Form N-550)
• Unexpired foreign passport with attached Employment Authorization
• Alien Registration Receipt Card with Photograph  (INS I-15 or I-551)

 (Also known as “Green Card”)

• Temporary Resident Card  (INS I-688A)
• Employment Authorization Card with Photograph  (INS I-688A)

Option Two: If you do not have one of the documents listed above and are a United States citizen, you are required to provide two documents, one to prove identity and one to prove that you are authorized to work in the United States. 

Identity: 

• State issued driver’s license with photo
• Voter’s registration card
• U.S. military card
• School I.D. with photo

Employment Authorization: 

• Original social security card
• Birth certificate issued by state, country, or municipal authority, bearing a seal or other certification
• Certificate of birth issued by the Department of State (FS-545)
• Unexpired INS Employment Authorization
• Unexpired Refugee Travel Document (INS I-571)
