
Dear New Student:
On behalf of the student
health center staff, welcome to Whittier College.
Although we hope you can complete your studies without
illness, you may need to visit the Student Health Center
on occasion. We offer primary care and health education
in an atmosphere of respect and confidentiality.
Please call ahead if you have a health problem or a
health-related matter to discuss.
IMPORTANT
INFORMATION-PLEASE
READ CAREFULLY! Complete and return the enclosed
health form(s) to the Admissions Office in the enclosed
envelope as soon as possible. Because it is
important for individual and community health, students
who do not return the completed forms will not be able
to register for Fall Semester classes.
Your primary physician
or healthcare provider is required to fill out the
vaccine requirement form.
Do not ask your physician
to send records to us without this instruction sheet and
the immunization form. Before mailing, please review
forms completed by your doctor for compliance. Make
sure forms are legible. Faxed records are often
unreadable; therefore mailing back the original copy is
the most efficient way to preserve your records.
Make a photocopy for your records.
INSTRUCTIONS:

Make an appointment
with your health care provider as soon as you
receive these forms.

In compliance with
California State Law, Whittier College requires that
all students provide documentation regarding their
informed decision of whether or not to immunize
against the Meningococcal disease. The Advisory
Committee on Immunization practices and the American
College Health Association strongly recommend that
college students be immunized against Meningitis.

You will need a
tuberculin skin test (PPD Mantoux) within 12 months
of the start of the semester. DO NOT mail your
health forms before your PPD is read! The result
must be marked on the form.

Tetnas (Td or Tdap)
must have been given within the last 10 years.
-
Hepatitis B vaccine
(three doses) is required of all students. If you
begin your vaccine before attending Whittier, you
may complete the series at the Student Health
Center.

Students MUST have
proof of two doses of MMR vaccine.

NCAA athletes are
required to complete an Athletics Participation
Screening History. A form is included in this
binder and must be returned to the Admissions
Office.
