 
Student Health Insurance Form
Undergraduate Students
All Whittier College undergraduate students can use the
health services, regardless of their insurance carrier.
Undergraduate students automatically pay Student Health
Services fees as a part of tuition and fees assessed by
the business office at the beginning of each semester.
Graduate and Law Students
A yearly fee is assessed to graduate and law students
wishing to use the services of the health center at the
time of the initial visit.
Health
Insurance
All
students enrolled at Whittier College must carry health
insurance. If you are not covered on an insurance plan
through your family or workplace, Whittier College offers
a supplemental insurance program.
If you
have chosen to maintain health insurance outside of the
college it is necessary for you to sign a waiver prior to
or during registration. If you have chosen to accept the
health insurance offered through the college but are
uncertain of the terms and coverage, please come by
Student Health Services and pick up a brochure that will
explain the coverage offered by the health insurance.

If you
carry Student Health Insurance it is very important that
you carry your student insurance identification card at
all times.

There is
a $200.00 accumulative deductible fee, per school year,
per student.

There
may be fees associated with lab tests for such things as
blood work, pap tests, sexually transmitted disease
screening and urinalysis ordered by the Student Health
Center.

The lab
will bill you for "nonessential" tests and you are
responsible for the payment of these billings (i.e.
blood test required for employment).

"Essential" lab tests are covered by the college
insurance provider (i.e. lab test required due to
illness or indicated by symptoms).

If you
receive a billing for health care services, you may
submit your claim using a "Claims Transmittal Form" to
the insurance company for consideration of payment.

If you
are covered by the student health insurance and need to
locate a preferred provider you can call 1-800-8572-5508
(M-F 8:00am-4:00pm est).

If you
seek medical care outside of Student Health Services,
are covered by student health insurance and utilize a
preferred provider, the plan covers 90% of eligible
services and you are responsible for paying the
remaining 10%. If you utilize the services of a
non-preferred provider, the insurance plan will cover
70% of the eligible services and you are responsible for
30%.
