Student Request for Recommendation letter

To the student:  I am always willing to write and send recommendation letters for deserving students.  Before you ask me to write a letter for you, you should seriously consider a few things.  First and most important is that any letter I write will be honest and accurate.  If you got a C- in my class, I'll probably say this in the letter.  If you show up to class drunk and disruptive every Tuesday, I'll probably mention this too.  If you worked extremely hard, or got a good grade, and are just a great person to be around, I'll certainly include this too.  If you think that my honesty might decrease your chances of getting into grad school, or being awarded that scholarship, it might be better to ask others to write more supportive letters.  But if I think that you would do well in grad school, or make good use of scholarship money, I'll write you a letter which will leave no doubt in the recipients mind that you're a quality student more than deserving of their consideration. 

In asking me for a letter, I'll need to know as much as possible about you and the people who will be receiving your letter.  Please include as much information as you can ... below I list the essential information I'll need:

The following information MUST be included with your request:   

    The name of the individual and/or organization receiving the letter

    The complete address of the individual and/or organization receiving the letter

    The deadline before which the letter must be received

    A description of what you're applying for (graduate school, financial support, etc.)

    What is the recipient looking for in you or your performance in college? 

    Are there things you feel I should highlight?

    Why should you be accepted (for grad school, financial aid, or whatever) over someone else?

    A complete list of courses you've had with me

    Your contact information ... email is probably best.  This is in case I need to reach you as I write your letter.
